coffin shaped
3 oz size
no major cracks or chips
embossed on 3 panels "USE WITH CAUTION", "CARBOLIC ACID" and "POISON"
bottom is embossed "O.C.P. ACID CARB"

There are some bubbles in the glass. There's also some residue inside the bottle, giving it a bit of a cloudy look. I'm sure this would clean up easily though.
