I know for sure that it's a Hazel-Atlas, and I believe the K means it was produced in the Zanesville plant, but I can't find any more information than that. There are two books available that have all the information you need to find the date and purpose, and that's probably why I can't find free copies of the old Hazel-Atlas catalogs. I'll let you know if I find anything else.
