It's tough to read but it looks like it says Loudoun Dairy 4515 Wisconsin Ave. Horner & Burchell. This is a gill size bottle. I have some other Loudoun Dairy bottles (larger sizes) but with a different address.

Any info would be appreicated ... thanks!

I think this milk is from DC, but not entirely sure. The little 1/4 pints are usually tough to find from most dairies.
