Yours is, I believe, the 1971 Commemorative.

"The Maxwell House Hotel in Nashville, Tennessee was famous for more than just its coffee.

Jack Daniel's Tennessee Whiskey was the exclusive whiskey served by this grand hotel. Lem Motlow, Jack Daniel's nephew, designed this special bottle for the hotel's famous Oak Bar. The half-gallon reproduction of this bottle was made available in 1971, and a 1.5-liter version was produced for international markets in 1995." From Jack Daniels.
